Housing and Dining Accommodations

Southwest Minnesota State University students with disabilities may request housing accommodations by contacting Accessibility Services.

Step 1:  Submit an on-campus Housing Application to Residential Life.

Step 2:  Ask your healthcare provider to complete the Health Care Form for Students Requesting Housing Accommodations.

  • The completed form should be signed and stamped by the provider and returned to Accessibility Services.

Emotional Support Animals (ESA)

Dining

Step 1:  Schedule a meeting with the Director of Dining Services, Seth Johnson, at seth.johnson@compass-usa.com or (507) 537-7681 to determine if your food-related needs can be met through Dining Services. 

Step 2:  If you require additional accommodations related to eating, food, and/or allergies, please have your medical provider complete the Documentation of Disability and then schedule a meeting with Accessibility Services at (507) 537-6492.
